U.S. Steel identified valve failure as the preliminary cause of a deadly explosion at its Clairton Coke Plant. The blast killed two workers & injured over ten during maintenance preparation. Pressure buildup in a gas valve led to failure & coke oven gas ignition. This marks the first facility death since 2014.
